Is this HIPAA compliant?

Interview does not receive or store any session content, so there is no business associate relationship to sign. The recording lives on your phone, and protecting it is the same as protecting the phone: a passcode, and an encrypted backup if you use iCloud Backup. This is not legal advice. Check your own obligations.

Do I need the client's consent?

Yes, if you record a session. Recording consent rules vary by state and country. Dictating your own note afterward needs no consent.

Does it need an internet connection?

No. Everything runs on the phone.
